Barriers to uploading diabetes technology at home

van Boxel Elizabeth , Trevelyan Nicola

Background: Our department cares for 273 children with type 1 diabetes (T1DM). All T1DM patients are issued with an uploadable glucose meter from diagnosis. 101 patients (37%) use insulin pump therapy and 126 (46%) use a continuous glucose monitoring device. Anecdotally we noted many patients were not uploading their data prior to coming to clinic. This contributed to reduced patient flow through diabetes clinics and also to reduced patient and carer engagement with their diab...

Increased diabetic ketoacidosis at presentation of type 1 diabetes mellitus – A result of the COVID-19 pandemic or longer-term increasing trend?

Moon Rebecca , van Boxel Elizabeth , Berg Elizabeth , Trevelyan Nicola

Background: Reports from several countries have suggested an increased incidence of diabetic ketoacidosis (DKA) at presentation of type 1 diabetes mellitus (T1DM) during the COVID-19 pandemic. Published data on this from the United Kingdom is sparse. We examined the frequency and severity of DKA at diagnosis of T1DM in children presenting to a university hospital during the first year of the pandemic in comparison to preceding years. Methods: In England, ...

Does diabetic ketoacidosis at diagnosis of type 1 diabetes mellitus affect anti tissue transglutaminase immunoglobulin A (tTG-IgA) levels?

Moon Rebecca , van Boxel Elizabeth , Berg Elizabeth , Trevelyan Nicola

Background: Current guidance recommends screening for coeliac disease (CD) at diagnosis of type 1 diabetes mellitus (T1DM) using measurement of serum anti tissue transglutaminase immunoglobulin A (tTG-IgA). It is recognized that tTG-IgA levels can fluctuate and there are reports of transient rises in tTG-IgA levels in response to acute stress, such as infection and acute coronary syndrome. The effect of diabetic ketoacidosis (DKA) on tTG-IgA has not been reported. We investiga...

A single-centre evaluation of telemedicine consultation and associated CO2 emissions for children and young people with diabetes

Andrews Edward , Catton Tim , Odeleye Esther , Kumaran Anitha , Trevelyan Nicola

Background: Telemedicine use has increased rapidly during the COVID-19 pandemic, replacing many face-to-face (FTF) consultations. FTF consultations are associated with increased CO2 emissions (CO2em) from travel to clinics. This study evaluates the triple bottom line of children and young people’s (CYP) and parent or guardian experience of a new telemedicine service, estimated CO2em saving from reduced travel and cost saving to families. Methods: Dat...

Use of U200 insulin degludec (Tresiba) and metformin in an adolescent with Type-1 diabetes-mellitus

Bhushan Arya Ved , Newland-Jones Philip , Trevelyan Nicola

Background: Insulin dose requirements are higher during puberty and in overweight/obese individuals with type 1 diabetes mellitus (T1DM) due to insulin resistance. Through meta-analysis metformin has been shown to be beneficial as adjunctive therapy in T1DM adults independent of BMI. The large volumes of insulin required to administer higher insulin doses in insulin-resistant individuals have adverse effects on insulin absorption. Insulin degludec (Tresiba) U200 preparation al...

Review of guidelines for the management of children and adolescent with diabetes requiring surgery in three regions of England

Anuar Azriyanti , Shailendra Rajput , Nicola Trevelyan , Edge Julie

Introduction: The management of diabetes during surgery in children is not evidence-based although the ISPAD has produced consensus guidelines.Aims: To explore the variability of guidelines in three regional diabetes networks in South West and South Central England and to compare them to the current ISPAD 2009 Guidelines. Methods: Within an audit of in-patient care, a copy of the surgical guidelines was requested from 27 paediatric ...
